INTRODUCTORY LESSON QUESTIONS

INSTRUCTIONS FOR COMPLETING THE FOLLOWING FORM

If you have studied the Introduction Lesson, you are ready to complete the following form.  Here are the four easy steps:

STEP 1 Answer the 20 CHOICE QUESTIONS.  Each question is about a scripture discussed in the Lesson.  Simply choose the correct ending to the sentence, and show your choice by clicking in button A or B or C.  (See the EXAMPLE)
STEP 2 Answer 3 of the short DISCUSSION QUESTIONS.  We invite you to join in discussing the Lesson.  (If you wish to answer all 4 Discussion Questions you may; but only 3 are required.)  Since your Discussion answers are not used for grading, you can feel free to say what you really think.  By learning your views, your Study Helper may be able to answer your special questions and offer helpful suggestions from the Bible.
STEP 3 Fill in the INTRODUCTION TO STUDENT.  Students have many different customs about names and addresses.  Help us to keep your student records clear.
STEP 4 Submit your completed form.  Once you are sure you have all the questions answered and have completed the INTRODUCTION TO STUDENT section, submit you form by clicking on the "SUBMIT" button at the bottom of this page.
